Few people can maintain their original teeth whiteness without some form of treatment or intervention. Most individuals experience tooth discoloration over the years. The accumulation of some stains and yellowing is generally inevitable.

Many safe teeth whitening options exist. While some over-the-counter treatments can be less expensive, they are usually not as effective or as comfortable as what you get at the dentist’s office. Understanding the causes of tooth discoloration and the costs associated with teeth whitening can help you decide how to treat your discoloration.

Causes of Tooth Discoloration

For most people, tooth staining is a fact of life. This is because your teeth contain holes and grooves that absorb color from what you eat and drink. When this coloring builds up over time, darkening, yellowing, or white spots on your teeth often result.

There are three types of teeth staining, one of which is extrinsic, which primarily affects the enamel or surface of the tooth. Coffee, dark sodas and teas, red wine, and dark fruits can leave stains on tooth enamel, but they tend to be easier to remove.

People can also develop intrinsic staining, which occurs inside the tooth. Intrinsic discoloration typically results in a grayish appearance, and it is usually impervious to over-the-counter (OTC) whitening products. This discoloration can come from tooth decay, certain medications like antibiotics, consuming too much fluoride, or injury to the tooth. Also, some people are genetically predisposed to intrinsic staining.

Finally, tooth discoloration usually happens as we age. The older we get, the more the enamel wears away, showing more of the dentin, which is the inner, softer part of the tooth. This dentin exposure creates a yellow look.

Professional Teeth Whitening Cost

Teeth whitening solutions vary significantly by cost, delivery method, and results. At the most basic level, you can purchase mouthwash and toothpaste that offer whitening benefits. Such products cost $5-15 dollars, contain minimal amounts of peroxide, and pose a relatively low risk for people with sensitive teeth and gums. However, whitening toothpaste and mouthwash might not yield the long-term effect you desire.

You might also consider whitening strips, another type of OTC remedy. A package of strips can cost $30-100 and contain 5-15% peroxide. Simply place the strips over your bottom and top teeth and remove them after about 30 minutes. While they can yield noticeable results, some people are sensitive to the strips, which can be challenging to keep in place.

Other patients opt for LED kits, which you only need to use a few times a month. They cost between $50 and $300, and they offer the non-invasive use of LED light to remove more persistent stains. However, while LED kits provide relatively quick results, they can also cause moderate sensitivity in teeth. Also, scientific studies have found the LED light isn’t necessary.

Professional teeth whitening is also available for people who have stubborn stains or want quicker and longer-lasting results. Most professional procedures use more peroxide than at-home versions, which can cause temporary sensitivity. Dental whitening can cost $400-$1,000 but requires fewer treatments.

Many dentists offer KöR teeth whitening, a safer alternative to professional teeth bleaching and lasers, which can aggravate sensitive teeth and gums. KöR deals with discoloration by restoring oxygen in teeth. Also, unlike most tray-based gel treatments at the dentist, KöR contains peroxide that remains constantly refrigerated from shipment to application. Keeping the peroxide cool prevents it from deteriorating, making it a more effective teeth whitening treatment.

With KöR Whitening, there are four whitening plans the doctor will prescribe based on the type of stains present in the teeth. The basic plan is a KöR Home which is an at home system that can be used day (9% Hydrogen Peroxide) or night (16% Carbemide Peroxide). The next step us is KöR Max which adds an in-office application following the at home whitening (34% Hydremide Peroxide). Even more powerful is the KöR Ultra which adds an in-office treatment (13% Hydremide Peroxide) before at the at home application and another in-office application (34% Hydremide Peroxide) following the at home whitening. The final KöR system has been specially designed to treat deep tetracycline staining in the teeth. This is KöR Ultra-T which starts with in-office (13% Hydremide) followed by 6- weeks of at-home whitening and finalized with in-office treatment (34% Hydremide Peroxide). Each professional whitening treatment requires maintenance. Some will perform maintenance whitening one or twice a month some may do 1-5 treatments every 3 months. Others may perform maintenance teeth whitening once per week or once per year. Maintenance cycles vary greatly depending on the patients diet and teeth reactivity.

Benefits of Teeth Whitening at the Dentist

As previously mentioned, teeth whitening in the dentist’s office can aggravate tooth sensitivity in some patients. However, this effect is typically moderate and temporary. Also, professional whitening has advantages over OTC at-home treatments. In-office procedures are safe and cause virtually no damage to teeth and gums. Also, bleaching and other whitening options use carbamide peroxide or hydrogen peroxide to remove more difficult stains.

Another benefit of professional whitening is that it typically offers faster results. Most patients see enhanced whitening just one hour after their in-office treatment. In addition, the improved appearance lasts longer than OTC alternatives.

Also, OTC treatments are not customizable, meaning that they offer a one-size-fits-all approach. On the contrary, patients can receive professional whitening that fits their preferences and specific dental needs.

Professional teeth whitening benefits can be cost-prohibitive for some patients. Most dental insurance plans do not cover in-office teeth whitening and other types of cosmetic dentistry because they are not medically necessary procedures like root canals, cavity fillings, or crowns. However, patients wishing to pursue professional teeth whitening might ask their dentists about payment plans that can make whitening more affordable. Teeth whitening is by far the most affordable form of cosmetic dentistry and can render fabulous results if done correctly.

Tooth wear tends to become more prevalent with age. Worn-down teeth can lead to various cosmetic and oral health problems such as yellowed teeth, tooth sensitivity, gaps between teeth, and jaw aches. However, many treatment options are available for tooth wear that can improve your smile and reduce any pain or discomfort associated with worn-down teeth.

Results from a 2009 study published in the International Journal of Prosthodontics found that the percentage of adults with severe tooth wear increases from 3% at the age of 20 to 17% at the age of 70. However, many other factors can contribute to tooth wear earlier in life, such as teeth grinding, physical abrasion, and acidic foods and beverages.

Here is a closer look at how to fix worn-down teeth, common causes, and prevention and treatment options that may be available from your dentist.

What Causes Your Teeth to Wear Down?

Worn-down teeth can be caused by one or several factors. Your dentist will ask you a series of questions during your appointment in an effort to identify the root cause of your worn-down teeth. This will help both you and your dentist develop prevention strategies and determine the best treatment solution.

The following are common causes of worn-down teeth.

Bruxism

Bruxism is the medical term for clenching or grinding teeth. It can happen anytime during the day or at night, though bruxism during sleep is often more difficult to manage and control. According to results from a 2016 study published in Sleep, bruxism during sleep affects an estimated 8.6% of the general population.

Bruxism can be caused by a wide range of factors, including stress, anxiety, diet, posture, and misaligned teeth. Common symptoms of bruxism are headache, pain or soreness in the jaw, insomnia, eating disorders, earache, and depression. When left untreated, bruxism can cause tooth damage and temporomandibular joint and muscle disorders, which are disorders that affect the chewing muscles and joints that connect the lower jaw to the skull.

What type of damage occurs as a result of teeth grinding? Teeth that grind together can cause friction, which gradually rubs away the enamel. Additionally, clenching the jaw can cause tiny cracks in tooth enamel. Over time, as tooth enamel wears away, teeth can become more sensitive to hot and cold foods and beverages, and more susceptible to rot and decay. As the back teeth wear down, the front teeth hit harder leading to excessive wear, loosening teeth and spaces or gaps between the front teeth.

Abrasion

Using your teeth as a tool to open packaging, or frequently chewing on hard nonfood items such as fingernails, pens, and pencils, can also wear down teeth. These behaviors cause physical abrasion that can wear down enamel, similar to how sandpaper wears down wood.

Physical abrasion can also occur when you brush your teeth roughly using a toothbrush with hard or stiff bristles. Many dentists recommend using toothbrushes with softer bristles to reduce physical abrasion and prevent the erosion of tooth enamel.

Exposure to Acids

Tooth wear can occur with regular exposure to acids such as that from vomit and certain foods and beverages. The American Dental Association reports that dental erosion and tooth wear caused by acids affects an estimated 45.9% of children and 80% of adults in the United States.

Acids from vomit, foods, and beverages can weaken tooth enamel that wears away easily when brushing teeth, chewing hard foods, or grinding teeth. Sodas, sugary drinks, coffee, tomatoes, and citrus fruits are often highly acidic. Health conditions associated with frequent vomiting such as anorexia, bulimia, and substance use disorders can also expose teeth to high amounts of acid.

Certain Teeth-Whitening Treatments

Studies have demonstrated that aggressive teeth-whitening treatments can wear away enamel and contribute to worn-down teeth. However, teeth whitening can be effective and safe for teeth when performed by an experienced dentist. If you are interested in brightening your teeth, ask your dentist about in-office whitening treatments that will not erode enamel.

What Problems Are Caused by Worn-Down Teeth?

Worn-down teeth can cause aesthetic problems and serious oral health issues that require professional dental treatment.

The following are problems commonly associated with worn-down teeth.

Yellowed Teeth

Tooth enamel is white; however, the layer of your teeth below enamel—known as dentin—is yellow. Your teeth may gradually become more yellow as the enamel on top of dentin wears away due to bruxism, abrasion, or a diet high in acidic foods.

See-Through Teeth

“Why are my teeth see-through?” is a common question many patients ask their dentists. Factors that contribute to see-through teeth are aggressive bleaching and whitening treatments, acid erosion, and celiac disease—the latter of which is associated with poor development of tooth enamel.

Tooth Sensitivity

When the enamel on your teeth is worn away, the nerve endings in dentin are exposed to every substance you put in your mouth. Worn-down teeth can make you extremely sensitive to certain foods and drinks, and may even cause pain when brushing your teeth.

Headaches and Jaw Aches

Teeth that become worn down can lead to alignment problems with your bite and jaw to cause strain, headaches, and jaw aches. You may also tend to bite harder or chew differently to compensate for problems with tooth alignment.

Gaps, Spaces and Loose Teeth

When the back teeth wear down and become shorter, the consequences to the front teeth can be devastating. It is termed, secondary occlusal trauma and it can lead to loosening of the front teeth, bone loss, gaps between the teeth and even tooth loss.

How Do I Stop My Teeth from Wearing Down?

The key to preventing your teeth from wearing down is to identify the root cause behind why they are being worn down in the first place and to manage it appropriately. For instance, if your teeth are worn down due to consuming high amounts of acidic foods and beverages, you can make changes to your diet to reduce your intake of these foods.

If you suffer from bruxism and have been grinding your teeth, solutions include wearing a mouth guard at night while you sleep, avoiding alcohol and caffeine, and practicing stress management. If you have bruxism, your dentist can give you additional tips on how to stop grinding your teeth.

What Are Treatment Options for Worn-Down Teeth?

There are several treatment approaches for worn-down teeth, including dental veneers and crowns. Certain dental products can help strengthen and repair your enamel to reduce discomfort, pain, and future dental problems. Tooth enamel cannot be naturally regenerated; however, enamel can be remineralized with specialized toothpaste and treatments but only if enamel still exists.

How can you tell whether your enamel is gone? Yellowed teeth, tooth sensitivity, and changes in tooth shape are the most common signs of worn-down enamel and teeth.

Your dentist can perform a thorough examination of your mouth and recommend the best treatments for restoring and fixing worn-down teeth. If you have bruxism, your dentist can discuss your options for teeth grinding damage repair.

Below are common dental procedures for worn-down teeth.

Dental Bonding

Dental bonding is a procedure in which a tooth-colored composite material is applied to the surface of teeth to cover worn surfaces and improve the appearance of teeth. Bonding may also reduce tooth sensitivity and address problems with bite alignment. Your dentist may recommend dental bonding if you have minor to moderate enamel erosion in isolated spots.

Porcelain Veneers

Porcelain veneers are thin layers of porcelain that are bonded to the surface of teeth to improve their colors, shapes, and sizes cosmetically. Veneers can also strengthen the surface of teeth to reduce sensitivity and make them less susceptible to damage and discoloration.

Porcelain Crowns

If you have several teeth that are severely worn, your dentist may recommend covering those teeth with porcelain crowns. A porcelain crown can cover an entire tooth to prevent cracks in enamel from spreading and protect the tooth from additional damage.

Charcoal Toothpaste – An Oxymoron?

Charcoal dentifrices, including toothpastes, gels, liquids and toothpowders, have become a rapidly emerging fad promising whiter teeth by using a more natural ingredient – activated charcoal. It’s ironic that people are turning to a toothpaste that looks like black mud for whiter teeth and a healthier, brighter smile.

Many activated charcoal products for teeth are trending on social media sites like Instagram and on natural health and home remedy websites and blogs. While a lot of online influencers claim charcoal toothpastes work, and post pictures of themselves using it, there are health risks involved in using activated charcoal as part of an oral health regimen. Whether you’re using it now or are considering it, you may want to learn what dental professionals are saying about this alternative toothpaste, and find out if it really works and what risks are involved.

According to the US National Library of Medicine, “Common charcoal is made from peat, coal, wood, coconut shell, or petroleum. “Activated charcoal” is similar to common charcoal, but is made especially for use as a medicine. To make activated charcoal, manufacturers heat common charcoal in the presence of a gas that causes the charcoal to develop lots of internal spaces or “pores.” These pores help activated charcoal “trap” chemicals.

Activated charcoal is used to treat poisonings, reduce intestinal gas (flatulence), lower cholesterol levels, prevent hangover, and treat bile flow problems (cholestasis) during pregnancy.”

Origins of Charcoal Toothpaste

Most of us are familiar with the common medical use of activated charcoal to treat poison, including toothpaste overdose! Charcoal is known to absorb some toxins and keep them from spreading in the body. It is a cheap and natural medicine when used that way. People have been using a powdered form of charcoal as a key ingredient in toothpaste dating back to Roman times and in homemade toothpastes since the 19th century, with the idea that the porousness of charcoal would help scrub teeth clean. In terms of whitening teeth, which is associated with cleaner teeth and a better-looking smile, the charcoal toothpaste is thought to erase stains by trapping surface debris and impurities as well as absorb harmful bacteria, resulting in a healthier cleaner mouth. Makes sense, right? Not so fast…

A Professional Opinion of Charcoal Toothpastes

As for its use as toothpaste, the American Dental Association (ADA) cautions that while it may take a while to notice the negative effects, ultimately, activated charcoal is abrasive and will deteriorate the tooth enamel, something that cannot be restored once it is gone. When this happens, that whiter, brighter look that people are going for is replaced by the yellowish layer underneath the enamel, called dentin. So not only does it erode precious tooth enamel, the result is the opposite of what people are using it for.

Another risk is that only using charcoal toothpaste may not help prevent cavities the way other toothpastes do because they lack other important and active ingredients like fluoride. Also the abrasiveness of the charcoal can roughen the texture of the teeth making it more likely for bacteria to stick to the surface, which can lead to an increase in plaque build-up, putting you at risk for more cavities and periodontal disease (U I Pertiwi et al 2017 J. Phys.: Conf. Ser. 884 012002).

A review called, Charcoal and charcoal-based dentifrices, published by The Journal of the American Dental Association states, “Dental clinicians should advise their patients to be cautious when using charcoal and charcoal-based dentifrices with unproven claims of efficacy and safety.”

Other Ways to Whiten Your Teeth

Brushing your teeth with activated charcoal toothpaste is not only ineffective, it can be harmful. So what are some better, healthier alternatives? One of the easiest and least expensive ways to whiten teeth is using a whitening toothpaste that contains hydrogen peroxide. We at Sarasota Dentistry can talk to you about the different products on the market and also the variety of teeth whitening treatments, including other things you can do yourself at home, or you may opt for an in-office teeth whitening treatment. Factors to discuss are how dramatic you want the results to be and how quick, as well as your budget.

More studies need to be conducted on charcoal toothpaste, but the consensus so far in the dental field is that it is causing more harm than good. We invite you to talk to us about the claims you’re seeing on websites and social media about oral care and teeth whitening so we can give you our professional opinion and add to the conversation so you can make the best, most informed choices possible.

Should you choose professional teeth whitening via bleaching trays, or should you pay for laser teeth whitening? It’s a choice that has a big effect on your wallet as well as your smile.

How Does Bleaching Work?

When you have your teeth bleached, the dentist applies a hydrogen peroxide or carbamide peroxide paste to your teeth. This peroxide paste is placed in bleaching trays, which apply the paste where you want it, on the enamel of your teeth.

Once in place, the peroxide paste releases molecules of oxygen that work their way into the discolored sections of your tooth enamel and break down the discolorations and stains chemically.

Typically, your dentist will give you a fitted bleaching tray, and you’ll sleep with it in place or wear it from 30 minutes to 2 hours. You continue this daily for anywhere from a few days to a few weeks, depending on how much bleaching your teeth need.

How Does Laser Whitening Work?

Many people have the idea that with laser whitening, the lasers blast the dirt off your teeth. Unfortunately, this sci-fi notion isn’t true at all.

Remember how peroxide paste breaks down stains on your teeth’s enamel when you use bleaching trays? Well, you’re still using bleaching trays when you opt for laser whitening. All the lasers do is heat the oxygen in the peroxide paste to speed up the breakdown of the discoloration.

The dentist applies either a carbon dioxide (CO2) or an argon laser to the bleaching trays to hasten the chemical reaction that the bleach has already started. By speeding up the bleaching process, laser teeth whitening may be able to remove stains on your teeth with a series of 30-minute sessions, which are typically spread out at a rate of one every 10 days or so.

While some people choose laser whitening to avoid having to sleep with bleaching trays in their mouths, in some cases, dentists may choose to augment laser whitening with nightly-use bleaching trays, wiping out this potential advantage.

Bleaching is the simpler of the two types of professional teeth whitening. Given that both processes achieve the same results, you

have to think about other factors when deciding which you prefer. Consider these pros and cons while making your choice.

Pros of Bleaching

Because your dentist gives you individually created bleaching trays, they match your unique dental structure exactly, so the bleaching paste ends up precisely where you need it most.

Since you bleach your teeth while you sleep, you don’t have to take time out of your schedule for the process. You can actually multi-task while you sleep!

If you use the stronger bleaching solution for the shorter time periods (30 minutes or less), you may experience less sensitivity.

The bleaching trays are thin, clear, and not invasive, making them comfortable for most people to wear.

Bleaching costs far, far less than laser whitening. In fact, the overall cost is about 90 percent less than the typical costs associated with laser whitening.

Annual maintenance for bleaching runs about $40 (only 3 percent of the costs of laser whitening), which is the cost of the carbamide peroxide gel syringes needed for a year’s worth of maintenance. Because your bleaching trays can be reused safely and successfully for two to five years, you don’t incur extra expenses for new equipment.

Cons of Bleaching

Bleaching requires more applications than does laser teeth whitening (though in most cases, you’ll reach your desired level of whiteness more quickly with bleaching, due to the gaps required between laser whitening treatments).

Some people aren’t comfortable using the bleaching trays or aren’t willing to wait a few weeks to see results.

Like bleaching, laser whitening also has many pros and cons to consider before you commit to it.

The Pros of Laser Whitening

Laser whitening can usually be completed with a total of two to four treatments (though you may require more if your teeth are extremely discolored).

Each whitening session takes only 15 to 60 minutes.

You’re likely to see some results right away, which can be encouraging. In addition, the immediate results may be more dramatic than they are typically with bleaching trays.

If you discover that your teeth are sensitive to the use of lasers, you can take comfort knowing your treatments are performed under medical supervision to minimize any potential negative reactions or discomfort.

The Cons of Laser Whitening

SENSITIVITY!

Although the big plus of laser whitening is its speed, you actually have to spread the sessions out, with a gap of 10 to 14 days between each one. This means it could actually take up to two months to complete your treatment.

You have to take time out of your day to go to the dentist’s office for each treatment. If you have a complicated schedule or are unable to take time off from work as easily as you would like, this could mean delaying your treatments even further.

You have to watch your diet for the first 48 hours after a teeth whitening session — and some dentists may ask you to be careful for a full week. Eating colored foods — think coffee, tea, wine, red meat, almost all fruits, tomato sauce, soy sauce, and chocolate — can re-stain the teeth during this sensitive period. If sticking to all-white food is easy for you, this may not be an issue. Otherwise, you may have to drink everything through a straw or even put petroleum jelly on your teeth to try to protect them.

In many cases, dentists use a peroxide mixture that contains 35 to 44 percent hydrogen peroxide — far stronger than the 10-percent solutions that are approved by the American Dental Association for use in bleaching trays. While this concentration may not be unsafe, more studies are needed.

Some people learn their teeth are sensitive to the high concentration of peroxide used in laser whitening treatments, making the treatments potentially painful. People with this sensitivity tend to experience a feeling like electric shock in their teeth toward the end of each treatment.

The possibility that lasers may damage tooth pulp has not been ruled out. Some dentists believe lasers can erode the tooth’s enamel and weaken tooth pulp, perhaps permanently.

You don’t avoid bleaching trays when you opt for laser teeth whitening. Your dentist is likely to send you home with bleaching trays (not custom-fit to your mouth) that you must use with peroxide gel every day between your laser treatments.

The major issue for most people: Laser teeth whitening is very expensive. You should expect to pay at least $1,500 per year for the initial whitening and for the annual maintenance required. Because teeth whitening is a cosmetic procedure, no dental insurance will pay for it, so that money comes out of your own pocket.

The Bottom Line: Bleaching Vs. Laser Whitening

As you make your decision between bleaching and laser whitening, here’s the most important thing to remember: Laser whitening is bleaching, but on a slightly different timetable. In fact, the ADA has noted that studies show no long-term benefit for laser whitening when compared to bleaching.

Another consideration is that teeth whitening doesn’t work for everyone. Some people have teeth that are darker as a result of their genetic makeup. In these cases, all the whitening in the world may not make an obvious difference. While many people see an enormous difference in their appearance as a result of whitening (regardless of the method chosen), people who aren’t aware yet whether their teeth will respond to whitening may not want to make a huge investment in the process right up front.

If you’re like many people, the bottom line in the bleaching vs. laser teeth whitening debate is just that: the bottom line of your budget. Because laser teeth whitening is so expensive, it puts a stress on many people’s wallets. The other significant factor is the pain associated with laser teeth whitening or in-office teeth bleaching systems. At home whitening trays are much more tolerable when considering teeth sensitivity and can be controlled easily by removing the trays and placing a desensitizing paste in the tray and reapplying to the teeth.

Ultimately, this decision is one you should make in conjunction with your dentist, keeping your budget and your timetable in mind. While ads for laser teeth whitening may tempt you to break out your credit card, take the time to sort through the facts and advantages of both processes before coming to a final decision.
